Guidelines and Expectations for Applicants:

Applicants are required to provide two forms of valid identification; acceptable examples include a government-issued passport or driver's license. This is essential to verify the identity of the person applying for assistance. If you are applying on behalf of someone else, you must present legal documentation confirming your status as the legal caregiver or guardian of the individual seeking financial assistance.

Please be aware that this application is subject to a review process. Approval will allow you to proceed to the subsequent phase of the application process. Should your application not be approved, you will not move forward to the next step.
